It seems to me that there are a couple of different categories we could talk about. The de facto assumption usually underlying this question is the guitar player as soloist, but there are other categories, too, worth mentioning.

Solo
Iommi
Murray/Smith
Hammett, on those early records
Hanneman/King

Rhythm
Hetfield, for the brute-force down-picking
John Christ

Songwriter
Warrior
Hanneman

I do wonder if the age of Guitar Player As God has come to an end. Perhaps it is just my personal taste, but I find myself mostly listening to music without guitar solos. (It could be that I never had the skill to play them myself. Laughing ). There was a time when they were almost obligatory, just as live guitar and drum solos were obligatory during arena-sized shows. And then it all seemed sort of calculated and dull. I'd rather see passion and collaboration live than widdiliness for widdiliness' sake.

Keeping it strictly metal and limiting it to those who have escaped mention thus far (in no particular order):
    Trey Azagthoth
    Bill Steer
    Ihsahn/Samoth
    Snorre Ruch
    Denis "Piggy" D'Amour

So many, but right now, Adrian Smith comes to mind. His solo in The Red and the Black on Maiden's Book of Souls was a high point for me and exemplifies everything he did- every second didn't need to be filled with 424545345 new notes. It was just about the feeling.

My favourite current 'Metal' guitarist is Matt Pike. Outside of metal my favourite is Kurt Vile. Two Americans.

Real:

Eddie van Halen

Old JP Duo / new too
Old Slayer / Holt is good too, but ...
All Maiden Duos/Trios
Megadave/Kirk/Hetfield
